Meadow View Health and Rehabilitation is a for-profit 120-bed nursing home facility located at 2203 East Mechanic Street, Harrisonville, MO, 64701. Meadow View offers medical care, transportation, laundry, and meals. The facility also has a 22-bed Alzheimer’s secured unit. According to ProPublica, Meadow View has accumulated 56 deficiencies in its inspection reports and has paid $139,000 in fines.

Deficiency Report – September 20, 2022
There were 26 deficiencies, and Meadow View was fined $120,978. Deficiencies included residents found wearing hospital gowns instead of normal clothing, staff yelling at residents during mealtimes, and staff failing to follow residents’ advanced care directives. The report asserted that Meadow View “failed to promote dignity and self-worth for three sampled residents.”
Deficiency Report – November 23, 2022
Among other deficiencies, a Meadow View resident with a known history of aggression and violence struck multiple fellow residents. Alarmingly, a review of the resident’s care plan shows that Meadow View put “no new measures . . . into place to prevent further abuse” after the reported incident.
Deficiency Report – March 14, 2023
This report indicates that a staff member befriended a resident and, through emotional manipulation, obtained money and a vehicle from them. The resident was made to believe that they were in a relationship with the staff member. When the staff member indicated they were only friends, the resident felt hurt, angry, and violated. The director of nursing was unaware of the situation until the state inspector conducted an investigation.
Deficiency Report – August 14, 2023
In another incident, a resident recorded rude and inappropriate comments by a staff member. The resident had an open wound on his leg and required assistance going to the restroom. Walking out of the bathroom, the staff member said, “You can do everything but wipe yourself.” They then claimed that their taxes paid for the resident’s care.
