About the Riverside Premier Rehabilitation and Healing Center
Riverside Nursing and Rehabilitation Center opened on April 4, 2007, at 4700 NW Cliffview Drive in Riverside, Missouri. The number to reach their office is 816.741.5105. The center offers various services, including short-term and long-term care, outpatient therapy, and specialized memory care. It has a special unit called “Sunshine Place” that caters exclusively to women with Alzheimer’s and other cognitive disorders.
Despite the facility’s numerous services, the Centers for Medicare and Medicaid Services have designated it a Special Focus Facility due to a history of deficiencies and harm to residents.

Another report cited a failure to employ staff who are licensed, certified, or registered in accordance with state laws.

In the October 2024 inspection report, the facility failed to develop and implement policies and procedures to prevent abuse, neglect, and theft. In another case, the facility did not provide sufficient food and fluids to maintain a resident’s health.

Over the past three years, the facility has been cited for two deficiencies related to infection control. The center has incurred fines of $277,170 and has faced three payment suspensions due to non-compliance.
