Bluebird Wellness and Rehabilitation currently has a 1-star rating on Medicare.gov, well below the national average. The facility’s record includes 150 health inspection citations over the last three years, including 42 in its most recent inspection. The national average is 9.5 citations per year.

Staffing deficiencies also factor into Bluebird’s low performance. The facility reports a 65.3% nurse turnover rate, compared to the national average of 47.5%. Bluebird’s registered nurses spend approximately 17 minutes with each resident daily, while the national average is 40 minutes.

Violations at Bluebird Wellness and Rehabilitation

Bluebird has accumulated numerous violations, many of which involve serious safety and care failures. These are some of the most severe deficiencies over the past few years: 

  • Ulcer care: In August 2024, the facility was cited for failed ulcer care and prevention. A similar inspection in June 2024 resulted in a fine of $231,153.
  • Protection against abuse: A March 2023 inspection resulted in a documented violation of a failure to protect residents from abuse, including physical, mental, and sexual, as well as physical punishment and neglect. 
  • Accident hazards: In August 2022, the facility was fined $46,322 for several violations, including failure to clear the area of hazards and provide adequate supervision to prevent accidents.

These are only some of the many documented complaints and deficiencies at Bluebird Wellness and Rehabilitation.

Why Bluebird Wellness and Rehabilitation Is an SFF Program Candidate

The Centers for Medicare & Medicaid Services (CMS) and State Medicaid Agencies inspect nursing homes regularly to determine if they provide the quality of care necessary to protect and promote residents’ health and safety. Based on health survey cycles and complaints, they generate a list of Special Focus Facilities, or SFFs, and provide them with support to help improve their quality of care. State Agencies then conduct full inspections of the facilities every six months and make recommendations based on their observations.
