Missouri Nursing Home Ratings

Health inspection ratings evaluate nursing home abuse history, cleanliness, medication management, infection treatment, and more. Missouri is currently behind on required inspections. According to the CMS, there are over 500 nursing homes in Missouri, and dozens have not been inspected in over two years. 

CMS explains its health inspection rating criteria as follows: “The health inspection score is calculated using each provider’s current health inspection and the last 2 prior inspections, as well as findings from the most recent 3 years of complaints or facility reported incident inspections, and infection control inspections.” The number and severity of citations—resolved and unresolved—also impact ratings.  

If you notice signs of nursing home abuse, such as unexplained weight loss or injury, changes in behavior or mood, or poor hygiene, you should take swift action to protect your loved one. Take the following steps if you believe your loved one is being abused in a nursing home:

Take Swift Action – Ensure the safety of your loved one. Consider calling 911 or seeking medical help right away if they are in immediate danger or require medical attention. If you suspect ongoing abuse but it’s not an emergency, ensure the resident’s immediate safety by moving them to a safe location.

Keep a Paper Trail – Document your concerns by keeping detailed notes of any signs or incidents that led you to suspect abuse or neglect. Include dates, times, descriptions of injuries or issues, and the names of individuals involved or witnesses.

Contact the nursing home management – Report your concerns to the nursing home’s management or administration. Provide them with the information you’ve documented and request an investigation into the matter.

Contact the appropriate authorities – You can report nursing home abuse or neglect to your state’s Adult Protective Services (APS) agency or Ombudsman Program. They are responsible for investigating elder abuse cases in nursing homes. You can also contact your local law enforcement agency to report abuse, especially if it involves criminal activity.

Preserve evidence – If possible, take photographs of any injuries, unsafe conditions, or neglected areas within the nursing home. Document any conversations or correspondence related to the abuse or neglect.

Seek medical attention – Ensure that your loved one receives proper medical care for any injuries or health issues resulting from abuse or neglect. Document all medical evaluations and treatments.

Notify other family members – Inform other family members or friends of your suspicions and actions taken. They may offer support and assistance during this difficult time.

Consider relocation – Depending on the severity of the abuse or neglect, you may need to consider moving your loved one to a safer facility.

Stay involved – Continue to visit and advocate for your loved one’s well-being. Frequent visits and communication can help ensure their safety and quality of care.
